The blackout period for the LUSU health and dental plan is now over. This means that your benefits card will now work using the direct pay method at pharmacies and dental offices.
The opt-out deadline has passed for the 2011-2012 academic year.
If you are a full-time undergraduate student enrolled in 4.0 credits or more by midnight on Friday September 23, 2011, you are enrolled in the LUSU Health & Dental Plan.
The opt-in Period for first semester has passed. There will be another opportunity to opt-in during the month of January.
LUSU Health and Dental Plan FAQ’s
1. How do I know if I am covered under the LUSU Health and Dental plan?
If you are a full-time undergraduate student who was enrolled in 4 full credit courses by September 23, 2011 then you are automatically enrolled in the LUSU health and dental plan.***
***You are not covered if you opted-out online by the advertised deadline in September.
1. How do I receive my benefits card?
If you have a benefits card from last year it will work again starting today. If you do not have a benefits card yet you can stop by the LUSU office to pick one up. Alternatively, you can print off a benefits card by visiting www.studentvip.ca/lusu and clicking on the ‘Forms and Downloads’ link.
2. How does the benefits card work?
You can show your benefits card when picking up prescriptions and at dental appointments. The information on your benefits card will allow pharmacies and dental offices to put through your claim electronically. This means that you will only have to pay the portion of the cost that is not covered under the LUSU health and dental plan.
3. What if I have paid for prescriptions or other services before I had my card.
You can print off a claim form from www.studentvip.ca/lusu and mail it into the insurance company along with your original receipts in order to be reimbursed.
4. Can I use my benefits card for all services covered under the LUSU health and dental plan?
No, the card is only eligible to be used at pharmacies and dental offices. For all other coverage on the plan (i.e. chiropractor, vision) you will be required to pay for the service in full and then send in your receipts with a completed claim form to the insurance company. Claim forms are available at www.lusu.ca. Simply click on the health and dental plan icon on the homepage and then click on the large green link that says ‘Claims’.
5. How do I find out what is covered?
Coverage details can be found by going to www.lusu.ca, clicking on the health and dental plan icon on the homepage and then clicking on the large green link that says ‘Coverage’.
